Statement from Dr David Ashbridge, Chief Executive Department of Health and Families:
"The Department of Health acknowledges that this has been a sad and difficult time for the family and friends of Mrs Winter and we would like to extend our sympathy to them.
"Points raised by the coroner will be addressed. I would add that we take matters of the coronial process very seriously and have always endeavoured to cooperate fully with the coroner's office.
"I will give serious consideration to the report content and the coroner's recommendations," said Dr Ashbridge.
