The Chronic Conditions Strategy Unit is part of the Health Development Branch in the NT Department of Health and Families and has a number of roles:
- Providing strategic advice on a range of issues, strategies and policies relating to chronic conditions, towards improving the health of all Territorians by reducing the incidence and impact of chronic conditions
- Working with key partners including the Australian Government, various NT Government departments, public and private health professionals, Aboriginal community controlled health services, non-government and community organisations, consumer representatives and education and research organisations, to build partnerships and develop a collaborative approach to chronic conditions prevention and management.
- Coordinating the promotion, implementation and monitoring of the Northern Territory Chronic Conditions prevention and Management Strategy 2010 - 2020.
- Contributing to strategies aimed at closing the gap between Aboriginal and non Aboriginal health, particularly in rural and remote communities of the NT.
- Working with a range of program areas within the Department to provide leadership and promote chronic conditions prevention and management.
- Facilitating the development of common understandings of the social determinants of health and the impact on chronic conditions
- Coordinating the Chronic Diseases Network
