Chronic Diseases Network (CDN)

The NT Chronic Diseases Network was set up in 1997 in response to the rising impact of chronic disease in the Northern Territory. The CDN aims to promote communication, collaboration, coordination and collective memory in the area of common chronic diseases affecting the NT population across the continuum of care. It now has approximately 1000 members from 90 government and non-government organisations across the NT and Australia. Read more ...
