Preventable Chronic Disease Program

The Preventable Chronic Disease (PCD)  team is located in Darwin, Nhulunbuy, Katherine and Alice Springs.  The team is lead by the Program Director and has two coordinators, based in the Darwin and Alice Springs locations. 

Activities of the PCD teams include:

  • Provide leadership in preventable chronic disease across the NT
  • Provide health professional education in preventable chronic diseases and well women's screening
  • Support health centre staff to manage preventable chronic disease and women's health systems in remote health centres including well person's screening, preventable chronic disease management planning, recall systems and follow up systems
  • Support community based health promotion activities in remote communities
  • Co-ordinate podiatry outreach service and the Preventable Chronic Disease Network
  • Support male health activities in remote communities
  • Develop NT wide strategy around health promotion and prevention

The Top End PCD team has five main program areas, which include the:
